Client Safety and Consent Form

I or my representative(s) agree to release and hold harmless fully Catherine "Cat" Gelinas from and against any and all claims or liability of whatsoever kind or nature arising out of or in connection with my session(s).*

I understand that taking any mood/physical altering substances before my session might hinder the effects of the session. I understand that sessions require a clear mind and the ability to discern emotions and physical sensations.*

I agree that Cat Gelinas is allowed to assist me in learning to develop and trust my innate intuition and to empower me to heal myself. *

I further understand sessions with Cat Gelinas are not a substitute for adequate medical or mental health care and I intend to remain under the care of my primary healthcare and/or mental health provider especially if I have a mental health diagnosis..*

I understand that I am 100% responsible for my own emotional and physical health, healing, and wellbeing - physical and mental. I also understand I have the ability to heal myself.*

I understand that if I intend any harm to myself or others, I will seek out the appropriate mental health provider and that Cat Gelinas does not provide services for that.*

I understand that the Practitioner, Cat Gelinas, will not accept responsibility for my decisions and will not make decisions for me. I am responsible for my own decisions regarding my health, nutrition, wellness, and any interventions I decide to try.*

I understand my identity and any information about me, whether I share it or she discovers it on their own, will be held in the strictest confidence, except when specifically required by law.*

I understand that the Practitioner will accept and honor an appointment cancellation request up to 24 hours before my scheduled appointment. She will consider honoring a request within 24 hours in a family or health emergency.*

I understand that full payment is required before services are delivered.*

I understand that there is a No Refund Policy.*

I understand that if I have a mental health diagnosis, I need to be under the care of the appropriate mental health provider and that Cat Gelinas is not a substitute.*
